Medical Social Worker

2600 White Plains Hospital Medical Center

Position Details City/State: White Plains, New York. Department: WPH Care Management_3. Work Shift: Day. Work Days: Mon-Fri, alternating weekends and holidays. Scheduled Hours: 9AM–5PM. Pay Range: $37.44–$56.17 per 75‑hour pay period. Job Summary SW Care Manager performs the initial comprehensive assessment on admission in accordance with the Care Management Department policy, screens all patients using established tools for high risk indicators to ensure high risk patient populations receive the appropriate supportive services for discharge to prevent readmission, and assesses all populations for potential discharge planning needs. Provides ongoing reassessment of needs throughout the hospital course. Essential Functions Understand and adhere to the WPH Performance Standards, Policies, and Behaviors. Complete a comprehensive initial assessment. Perform all activities for multidisciplinary care coordination at the intra‑hospital and inter‑hospital levels of care. Perform all transitional planning activities, including but not limited to: patient/caregiver education; arrangement of after‑care and commercial payer involvement; high‑utilizer patient root‑cause analysis. Communicate and collaborate on all relevant patient information with appropriate health team members. Ensure department goals of LOS reduction, readmission prevention, denial reduction, and satisfaction improvement are realized. Document per department requirements. Participate in performance improvement activities as needed. Perform all other duties as assigned. Qualifications Master’s degree required; 1–3 years of related experience preferred. Two years of experience in an acute care hospital setting or related setting preferred. Preferred proficiency in Electronic Medical Records (medium proficiency). Licensed Master Social Worker (LMSW) – New York State Board of Social Work – required upon hire. Licensed Clinical Social Worker (LCSW) – New York State Board of Social Work – required upon hire. PRI – Patient Review Instrument certification required within 90 days of hire. PHTC – PRI/Screen NYS certification required within 90 days of hire.
