Care Plus NJ is seeking a dedicated In-home Intercept Case Manager in Fair Lawn, NJ. This role involves providing individualized family sessions in home and community settings to keep youth safe and divert them from out-of-home placements. The successful candidate will manage a caseload of five families, conducting three face-to-face sessions weekly.
