Primary Care Nurse Practitioner Bay Ridge & Bensonhurst

A Long-Term Care (LTC) facility in Bensonhurst, Brooklyn and Bay Ridge, Brooklyn is seeking a full-time Primary Care Nurse Practitioner to provide high-quality, patient-centered care to a geriatric population. This role is ideal for an NP experienced in managing chronic and acute conditions in older adults within a structured LTC environment.

Compensation: $160,000+ annually Benefits: Health, dental, vision, malpractice insurance, and 3 weeks PTO Location: Long-Term Care facility in Bensonhurst & Bay Ridge, Brooklyn Schedule: Full-time, in-person

Key Responsibilities: Perform patient histories and physical exams for approximately 25 geriatric patients per day Diagnose and manage acute and chronic conditions common in older adults Order and interpret labs, imaging, and diagnostic tests Prescribe and adjust medications in accordance with NYS regulations Coordinate care and make referrals to specialists as needed Document all encounters thoroughly and accurately in the EHR Collaborate with interdisciplinary teams including nursing, therapy, social work, and administration Support continuity of care for long-term residents and subacute patients

Qualifications: Current Nurse Practitioner license in New York Board Certified 1+ years of geriatric NP experience required Strong clinical assessment and diagnostic skills Compassionate, patient-focused approach
