Family Practice/Primary Care Nurse Practitioner

• Full-time Psychiatric Nurse Practitioner position in the Chelsea area of NYC.• Work with homeless patients mostly single adult men.• Services include mental health care, employment support, substance abuse programs, and housing assistance (transitional & permanent).• Collaborate closely with a dynamic head nurse and team of clinicians to connect residents to needed services.• Role focuses on evaluations and care coordination rather than heavy medication management.• Future opportunity to manage a small caseload for meds and therapy.• Schedule: Three long days per week (option for a four-day workweek if preferred).
