Family Practice/Primary Care Nurse Practitioner

LOCUM Telehealth NP/PA 135150: Southeast, New York

• Must have an active NYS Medicaid ID number
• Must have at least 2 years (3600 hours) of experience in any setting to qualify
• Operating hours: Monday-Friday, 8 am- 7 pm; Saturdays and Sundays 10-6
• Responsible for reviewing and making recommendations on consumer assessments for long-term care services and supports.
• Assess cases presented and utilize clinical competencies to review and validate assessment outcomes.
• Work collaboratively to monitor and address clinical quality and gaps in care
