Limited Duration Case Manager Senior

Multnomah County

Overview This Limited Duration position provides case management services for children and adults with developmental disabilities in Multnomah County. The role works within the State Office of Seniors and People with Disabilities contracts, supporting individuals under individualized service plans to access medical, social, educational, transportation, and other critical services. Key Responsibilities Provide comprehensive case management for youth and adults eligible for DDSD services, including assessment of appropriateness, re‑evaluation, and development of individual service plans. Assist clients in accessing needed medical, social, educational, transportation, and other support services. Coordinate crisis services and protective services, including documentation of complaints and potential investigations. Coordinate services with other agencies such as adult and family services, Children's Services Division, Vocational Rehabilitation Division, Senior Services Division, Social Security Administration, Veterans Administration, Department of Education, etc. Monitor services/supports via home visits, provider visits, and phone calls to ensure clients receive appropriate and effective support. Complete and submit routine and special reports, including TXIX Waiver forms for assessment of level of functioning and support needed. Notify the State when a client is out of a funded facility for any reason. Advise and counsel consumers/families on placement, care/service plan choices, financial planning, and conflict resolution. Provide information on client rights and coordinate resolution of grievances. Liaise with educational systems to assist children in accessing appropriate educational services. Analyze client files in CYAP and Adult units, providing direction and technical assistance to services coordinators, DD management, and community members regarding behavior support standards and resources. Support team meetings and facilitate consensus to achieve positive outcomes. Qualifications Knowledge of case management methods, principles, techniques, and resources, including information/referral, assessment, interviewing, client advocacy, risk intervention, and resource utilization. Knowledge of available programs, public assistance providers, Medicaid, service agencies, protective services, referral centers, community residential resources, and crisis/emergency resources. Understanding of conflict resolution techniques, de‑escalation strategies, and ability to facilitate large team meetings. Ability to demonstrate sensitivity to diverse cultures and provide courteous customer service. Strong organizational and prioritization skills; able to meet deadlines independently or as part of a team with good judgment. Excellent verbal and written communication, decision‑making, and research skills, including the ability to analyze data and prepare reports. Knowledge of federal, state, and local laws, rules, regulations, policies, and procedures related to program scope and operations. Minimum education/experience: a bachelor's degree in behavioral science, social science, or a related field; or a bachelor's degree with one year of human services experience; or an associate's degree with two years of experience; or three years of human services experience. Must have a valid driver's license and pass a criminal background check.
