The Program of All-Inclusive Care for the Elderly (PACE) offers a variety of services, with many of them provided onsite at a PACE Center.
PACE is an alternative to nursing home care and – through an interdisciplinary care team (IDT) of physicians, nurse practitioners, nurses, social workers, therapists, van drivers, and aides – coordinates and provides preventive, primary, acute, and long-term care services, so older individuals can continue living in the community.

Primary Purpose
Provide direct primary medical care to all participants with the highest medical standards and ethics in a caring, nurturing atmosphere while keeping with Senior Care Partners P.A.C.E. mission, vision, and values.
Duties and Responsibilities
- Performs an initial comprehensive history and physical assessment on new enrollees, interval assessments on established participants (semi-annually, as condition necessitates or as requested) and documents in the medical record as required.
- Functions as a member of the Interdisciplinary Team (IDT), communicates participant changes to team members, and participates in IDT meetings.
- Assists in the development, implementation, and evaluation of the participant plan of care as described in Senior Care Partners P.A.C.E. policy.
- Evaluates and treats participants with episodic illnesses and documents in the medical record as required by policy.
- Provides, promotes, and educates participants and families on preventative health maintenance of their chronic medical conditions and actively participates in family conferences.
- Regulates and adjusts participant medications as needed and in a cost‑effective manner.
- Facilitates consultations with medical specialists as indicated and oversees care rendered.
- Collaborates effectively with primary (attending) medical management while an inpatient; Provides updates to the IDT on a regular basis; Judiciously utilizes specialty consultants; Coordinates discharge planning with the IDT and hospital discharge planners.
- Manages all of a participant's care if admitted to a skilled nursing facility (SNF) to include: Regularly visit the SNF for participant evaluation; Communicates with SNF staff via phone/fax as requested/required.
- Participates in committees, work groups as assigned, and Quality Improvement activities as required.
- Participates in the coordination of the participant’s 24-hour delivery of care services.
- Maintains current accurate documentation of health care services provided, coordinated or contracted.
- Must care, support, and collaborate with everyone involved in participant's care including the participant’s family using the patient-family centered care approach.
Supervisory Responsibilities
This position has no direct supervisory responsibilities. The Physician works collaboratively with Nurse Practitioners and other healthcare professionals as part of the interdisciplinary care team to support the delivery of patient-centered care.
Qualifications
For an MD/DO or NP Role
- Graduate of an accredited School of Medicine, College of Osteopathic Medicine, or accredited Nurse Practitioner program.
- Current licensure as a Physician (MD/DO) or Advanced Practice Registered Nurse (APRN) in the applicable state.
- Board certification in Internal Medicine, Family Medicine, Geriatric Medicine, or applicable Nurse Practitioner specialty certification preferred.
- Minimum of one year of experience working with frail elderly, long-term care, hospice, palliative care, or medically complex patient populations.
- Demonstrated ability to work collaboratively within an interdisciplinary team to manage complex patient care needs.
Specific Requirements
- Must possess a current, unencumbered, active license to practice as a MD or DO in the State of Michigan.
- Must possess a current, unencumbered, active DEA license.
- Current privileges or the ability to obtain and maintain staff privileges at Senior Care Partners P.A.C.E. contracted provider facilities
- Possess current CPR certification by AHA or willing and able to obtain as offered by Senior Care Partners P.A.C.E.
- Must be able to read, write, speak, and understand the English language and follow written and oral instructions.
- Must demonstrate necessary skills and knowledge as outlined in the position specific competency requirements.
- Must be knowledgeable of current medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care.
- Must be willing to seek out new methods and principles and be willing to incorporate them into existing medical practices.
- Must care, support, and collaborate with everyone involved in participant’s family using the patient-family centered care approach.
- Must possess the ability to make independent decisions when circumstances warrant such action.
- Must possess the ability to communicate tactfully with personnel, participants, family members, visitors, government agencies/personnel, and the public.
- Must have proven leadership experience and skills with the ability to get along harmoniously with others in a team environment.
- Must possess the ability to plan, organize, develop, implement, and interpret Senior Care Partners P.A.C.E. goals, objectives, policies and procedures, etc., that are necessary for providing quality care.
- Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to manage difficult participants.
- Must be knowledgeable of computers, data entry, output, system applications, etc.
- Must not pose a direct threat to the health or safety of other individuals in the workplace.
Work Environment
- Has a visible presence while working in the center and is subject to frequent interruptions.
- Has a working presence in the community and is involved with participants, family members, personnel, visitors, government agencies/personnel, etc., under all conditions/circumstances.
- Works beyond normal working hours and on weekends and holidays when necessary.
- Subject to callback during emergency conditions (e.g., severe weather, evacuation, disaster, etc.).
- Is subject to hostile and emotionally upset participants, family members, personnel, and visitors.
- Is subject to injury from falls, burns from equipment, odors, etc., throughout the workday, as well as reactions from dust, disinfectants, tobacco smoke, and other air contaminants.
- May be subject to exposure and handling of infectious waste, diseases, conditions, or hazardous chemicals, etc., including potential exposure to TB, AIDS, and/or the Hepatitis B virus.
- Communicates with staff, participants, caregivers, the board of Senior Care Partners P.A.C.E. and other organizations, as necessary.
Physical Demands

- Must be able to move intermittently throughout the workday.
- Must be able to cope with the mental and emotional stress of the position.
- Must possess sight/hearing senses or use prosthetics that will enable these senses to function so that the requirements of this position can be fully met.
- Must function independently, have flexibility, personal integrity, and the ability to work effectively with participants, personnel, and support agencies.
- Must meet the general health requirements set forth by the policies of Senior Care Partners P.A.C.E., which includes a medical physical examination.
- Must be able to relate to and work with the ill, disabled, elderly, emotionally upset, and at times hostile people within the Senior Care Partners P.A.C.E. Program and/or center.
- Must be able to lift twenty pounds.
- May be necessary to assist in the evacuation of participants during emergency and/or disaster situations.
Additional Requirements
- Must have reliable transportation, current valid Michigan driver’s license and car insurance.
- Must be 18 years of age or older.
- Must be fully vaccinated, or willing to be fully vaccinated against the following communicable diseases: Influenza, MMR, Varicella, Tdap/Td, Covid-19 and Hepatitis B.
- Must be fully vaccinated or willing to become vaccinated against: Influenza, MMR, Varicella, Tdap/Td, COVID-19, and Hepatitis B.
- Must meet CMS competency standards prior to working independently
- Comfortable working in varied home environments and all weather conditions
