Join a purpose‑driven, resident‑focused care team in a supportive environment Your Role Plan, organize, and direct the Social Services department to meet the psychosocial and emotional needs of all residents Conduct and document social service assessments and care plans, including MDS interviews and input Provide interventions to support residents’ mental health and well‑being Coordinate resident discharges (home health, DME, transfers to other facilities) Facilitate resident care conferences with families, residents, and the care team Lead and implement engaging activities tailored to residents’ interests and abilities Develop and maintain a monthly calendar of activities and community events Build positive relationships with residents, families, medical professionals, and local organizations Complete required documentation, forms, and reports in compliance with regulations Who We’re Looking For Two years of job‑related experience, preferably in long‑term care (other qualifying experience considered) Experience in social work, especially in skilled nursing or long‑term care settings Knowledge of the MDS process and care planning Strong organizational, communication, and computer skills A compassionate, professional team player passionate about helping others Detail‑oriented and dependable with a commitment to high‑quality care Benefits Competitive Pay Comprehensive Health, Dental, and Vision Insurance Paid Time Off (PTO) Daily Pay Option — access earned wages in real time Employer‑Sponsored Life Insurance Career Growth & Leadership Development Employee Assistance Fund — apply for grants or donate PTO to help fellow team members Ready for a Rewarding Career?
