Licensed Clinical Social Worker

We are seeking a Licensed Clinical Social Worker (LCSW) – Care Management/Discharge Planning for a 13-week acute care assignment. This role focuses on coordinating patient care, facilitating safe discharges, and ensuring effective care transitions across a variety of hospital units. The ideal candidate will have experience in acute care case management, strong discharge planning skills, and the ability to work collaboratively across multidisciplinary teams in a fast-paced hospital environment. Key Responsibilities Coordinate patient care and discharge planning across multiple acute care units Perform comprehensive assessments, including psychosocial and care coordination evaluations Develop and implement individualized care plans to ensure safe and timely discharges Collaborate with physicians, nurses, and interdisciplinary teams to optimize patient outcomes Arrange post-discharge services, including home care, durable medical equipment (DME), rehabilitation, and long-term care placement Facilitate referrals to internal and external resources, including community services and support programs Monitor patient progress and address barriers to discharge, including high-risk or complex cases Ensure accurate, timely, and compliant documentation within the electronic medical record system Participate in weekend rotations and float between units as required Required Qualifications Licensed Clinical Social Worker (LCSW) license in WA Minimum 1 year of acute care experience (case management/discharge planning preferred) Experience with care coordination and discharge planning in a hospital setting Active Washington State license (RN or applicable MSW license) BLS certification required from AHA Experience with the EPIC charting system Preferred Qualifications Diverse case management experience across Med-Surg, ICU, L&D, NICU, or Emergency Department Experience working with adult and geriatric populations Familiarity with community resources, insurance (Medicare/Medicaid), and post-acute care coordination 13-week contract position Shift is 8 hrs/day (7:30 am - 4 pm) Mon-Fri, 40 hrs/week Location is Everett, WA 98201 Locals are not accepted within 50 miles from facility Pay Range for Travelers: $2,000 - $2,040 per week Pride Health is an equal opportunity employer.
