Care Coordinator

Role: Full-time Care Coordinator supporting children aged 0–3 with developmental delays or disabilities within a family-centered, trauma-informed Early Intervention program. Key Responsibilities Partner with families, service providers, and community resources to support child development. Conduct functional assessments and develop individualized service plans. Coordinate and facilitate services aligned with care plans. Support families through education, advocacy, and collaboration. Maintain accurate documentation in compliance with regulations. Lead family-centered team meetings and collaborate with internal teams and community providers. Manage a flexible schedule, including occasional evening or weekend hours, with remote work flexibility after training. Meet productivity and service delivery standards. Qualifications & Skills Bachelor's degree in Social Work, Child Development, or related human services field. Preferred experience working with families with special needs. Valid driver’s license and reliable transportation. Strong communication, organizational, and documentation skills. Ability to work in home, office, and community settings, including responding effectively in crisis situations.
