Position Overview Home Health Physical Therapist in Alameda, CA – full‑time and part‑time openings. Flexible schedule; sign‑on bonus of $10,000 for full‑time PTs. About the Company Prohealth is a locally owned home health, hospice & palliative care agency serving over 12 counties in Northern California. We provide nursing, physical therapy, occupational therapy, speech therapy, and registered dietitian services to an average daily census of over 1800 patients. Job Responsibilities Assess and evaluate therapeutic, rehabilitative, and functional status and participate in the development of the total plan of care. Evaluate home environment and assess muscle strength, mobility, gait, ROM, and potential for rehabilitation. Initiate physical therapy programs and instruct other personnel and/or family caregivers in care phases and program goals. Identify patient and family/caregiver needs for other home‑health services and refer as necessary. Qualifications Valid PT license to practice in California. Current BLS/CPR certification.
