Monitor and document residents’ conditions, vital signs, and response to interventions; report changes promptly to the care team. Develop and participate in resident-centered care plans in collaboration with physicians, families, and interdisciplinary staff. Ensure accurate medication administration, charting, dosing verification, and adherence to regulatory standards and safety protocols. Support residents with activities of daily living, fall prevention strategies, wound care, and infection control practices; respond to emergencies per organizational policy. Maintain up-to-date documentation in the electronic health record and uphold privacy, dignity, and ethical standards in care delivery.
