Community Health Action of Staten Island
Community Health Action of Staten Island (CHASI) is seeking a Care Coordination Outreach Engagement Specialist to support HIV care coordination. You will conduct outreach with community partners, medical facilities and venues frequented by target populations to link individuals to CHASI programs and medical care. You will assist with appointment scheduling, accompany clients, collect data, and participate in team meetings to ensure services are delivered with compassion and accuracy. #J-18808-Ljbffr Community Health Action of Staten Island
