Description

• Assess patients in both inpatient and outpatient environments while crafting personalized treatment plans tailored to their unique needs.
• Deliver physical therapy interventions targeting mobility, strength, balance, and overall functional capability.
• Engage with nurses, physicians, case managers, and fellow rehabilitation experts to optimize patient outcomes.
• Instruct patients and their families on therapeutic goals, discharge preparation, and home exercise regimes.
• Maintain accurate documentation of assessments, treatments, and patient progress in alignment with facility protocols and regulatory standards.
• Participate in interdisciplinary conferences and care team rounds to ensure comprehensive and coordinated patient care.

Education

• A Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution is required.

Certifications

• Current state licensure as a Physical Therapist.
• CPR certification is preferred.
• Having specialized certifications (neurological, geriatric) is a plus, but not mandatory.
