Samaritan Recovery Community is a caring, long-standing nonprofit substance abuse treatment provider serving Nashville and Middle Tennessee. Located in the historic East Nashville area with spectacular views of the downtown skyline, Samaritan Recovery Community provides intensive residential treatment, transitional living, intensive outpatient, and outpatient services. We provide treatment services to men and women regardless of their ability to pay.

POSITION SUMMARY:

The IR Case Manager will assist clients at all phases of their treatment program. They will monitor client behaviors, assist clients making calls and scheduling appointments, work with the treatment team developing discharge plans for each client including transfer to Transitional Living, referrals, medical and mental health follow-ups, and present educational material to clients.

 

  ROLES AND RESPONSIBILITIES:

 

Case Management Responsibilities

 

Participate and interact in daily activities of the clients, to develop relationships and trust, as well as help to maintain Samaritan standards throughout the programming portion of the day. The intent is to ensure the clients are engaged in their recovery at all times which includes monitoring and redirecting behaviors, supervising phone calls, helping to transport clients between floors, and breaks and meal periods.

Assist in the development of the service plan for each client, which involves motivation of committed treatment, support clients in transition, and foster independence for lasting community involvement.

Assist treatment team with making aftercare referrals, including but not limited to: outpatient mental health programs, intensive outpatient programs, and sober living arrangements.

o Discuss with the Clinical Director weekly and discuss appropriate referrals to SRC’s Transitional Living program.

Aftercare role including developing essential life skills and establishing long term goals.

Coordinate with clients who have legal concerns, including those on state probation.

o Send weekly progress reports to Forensic Social Workers.

o Send admission/discharge information to concerned parties within the legal system.

Responsible for occasional transport of clients to community appointments in the area.

Connect with housing administrations, childcare facilities, advocacy groups.

Will be included in the IR Lecture/Seminar rotation

Effective communication with Admissions Clerks, Counselors and Nursing staff.

Utilize resources in the community to aid clients with obtaining driver’s license, social security card, birth certificates, food stamps, TNCare, and grants for sober living/housing, etc. 

Documentation of activities in approved systems-progress of each client’s participation is a key element to this position.

Other related duties as assigned.

REQUIREMENTS:

High School Diploma some college preferred

Driver’s License with “F” endorsement

Understanding of recovery process and treatment

Able to work both independently and with a team

PHYSICAL DEMANDS / WORK ENVIRONMENT:

Must be able to sit/stand for 8 hours.

Must be able to walk / stand for up 50%of shift

Walking, bending, light lifting up to 50 lbs.
