Role Description The Physical Therapist will provide in-person, house call therapy services to patients in the Stamford, CT area, focusing on assessment, treatment planning, and implementation of individualized rehabilitation programs. Day-to-day responsibilities include conducting comprehensive evaluations, developing evidence-based treatment plans, delivering hands-on interventions, and monitoring patient progress. The role involves close collaboration with other healthcare professionals, maintaining accurate documentation, and educating patients and caregivers on home exercise programs and safety strategies. This is a full-time, on-site position based in Stamford, CT, requiring travel to patients’ homes within the service area. The Physical Therapist will uphold high clinical standards and contribute to a patient-centered, outcomes-focused care model. Qualifications Strong foundation in Medicine and Physical Therapy, with the ability to integrate medical knowledge into therapeutic decision-making. Demonstrated expertise in Rehabilitation, including evaluation, treatment planning, and progression of therapy interventions. Proven skills in Patient Care, emphasizing safety, empathy, communication, and culturally sensitive practice. Experience or readiness to provide Home Care services, including house call visits and in-home functional assessments. Current Physical Therapist license (or eligibility for licensure) in the state of Connecticut. Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program. Strong documentation and time-management skills, with proficiency in electronic health records. Ability to work independently in the field while collaborating effectively with an interdisciplinary team.
