PATH (People Assisting The Homeless) is hiring an Outreach Case Manager for the Union Station location in Los Angeles, CA. The role involves street outreach, rapid linking to housing, health and supportive services, and collaboration with a multidisciplinary team. Minimum of two years’ outreach case management experience is required. Driving is required and the position is full-time with specific shift times.
