PATH (People Assisting the Homeless)
PATH (People Assisting The Homeless) seeks an Outreach Case Manager for the Metro Rail Outreach Team in Los Angeles, CA. The role conducts daily street outreach to connect people experiencing homelessness with housing, health, and supportive services within the Metro Los Angeles region. You will assess clients, develop ISP plans, guide them to housing options, coordinate with partner agencies, and provide advocacy for navigating community resources and landlord relationships. #J-18808-Ljbffr PATH (People Assisting the Homeless)
