Hospice Care Navigator - RN Liaison

Mainerafting

Mainerafting is seeking an experienced Hospice Clinical Liaison in Brewer, Maine. In this role, you will ensure effective intake processes, build relationships with referral sources, and educate patients about hospice services. The ideal candidate has a strong background in hospice care and must be a licensed RN with at least three years of relevant experience.
