Registered Nurse (RN) - Case Manager, Hospice - $75K-95K per year

Graham Healthcare Group is seeking a Registered Nurse (RN) Case Manager for a hospice position in Cheswick, Pennsylvania. This is an ongoing, full‑time, staff role of 40 hours per week, with an 8‑hour shift structure. Responsibilities Deliver compassionate, patient‑centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life. Perform comprehensive patient assessments, symptom and pain management, and individualized end‑of‑life care planning. Educate and support patients and families through the end‑of‑life journey, ensuring they feel informed, prepared, and deeply cared for. Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation. Work independently, but never alone. Travel to patient homes within assigned territory. Qualifications Active Registered Nurse (RN) license in the state of employment. Two years of RN experience, with hospice experience preferred. Current driver’s license and ability to spend approximately 20% of your day driving to and from patient locations. The low end of the range includes compensation related to salary and first‑year success bonus. The high end of the range is inclusive of productivity bonuses and additional shift work that is paid at a premium (after hours, weekends, on‑call).
