Description:

The Registered Nurse implements professional nursing practice centered on the patient as a whole person. The role focuses on creating a caring environment that promotes healing and wellness. Nursing practices are based on evidence, current research, hospital policies, regulatory standards, and professional guidelines. Clinical decision-making incorporates all four phases of the nursing process, critical thinking, and creativity. Nursing excellence is promoted through helping relationships among nursing staff and across other disciplines. Responsibilities include coordinating, ensuring, and providing direct patient care activities. This includes provision of nursing rounds, assessments, and treatments. Facilitating admission, transfer, and discharge activities is key. Patient and family-centered education, care coordination, and individualized treatment planning are essential. Assessment of patient needs includes performing and documenting comprehensive assessments in accordance with scope of practice and hospital policies. Consideration of the patient's culture, age, and developmental level is crucial. Identifying patient risk factors and presenting complaints is necessary. Obtaining patient health information from family or others as necessary is part of the role. Collecting objective and subjective data is required. Verifying initial admission assessments, if obtained by other team members, is needed. Developing a problem list and prioritizing patient needs based on input from the inter-professional team is important. Designing actions and interventions relevant to the patient's culture and age is essential. Determining desired outcomes in coordination with the patient and family according to the medical plan of care is required. Fully implementing patient care therapies and procedures with empowerment and clinical autonomy within the scope of practice is a key responsibility. Explaining procedures, treatments, and processes to the patient and family is necessary. Initiating action to alleviate patient stress and discomfort is an important aspect of care. Recognizing, responding to, and communicating changes in patient condition appropriately is a critical skill. Conducting and documenting ongoing assessments and responses to procedures and therapies using the nursing process is required. Delegating tasks appropriately and overseeing team members in the performance of activities regarding patient care is part of the role. Monitoring the accuracy and completeness of delegated patient care provided by other team members is essential. Identifying and addressing educational needs of patients related to prioritized problems according to their level of comprehension is necessary. Facilitating the patient discharge process by executing discharge orders in a timely manner is a critical responsibility. Providing input into patient/nurse assignments in consideration of available staffing, unit activity, and patient acuity is required. Demonstrating active participation in unit-based activities in support of organizational values and priorities is essential. Promoting a positive group dynamic and healing environment through words and actions is vital. Assuring nursing excellence in accordance with accepted standards and evidence-based practice is crucial. Actively participating in department and unit quality improvement activities and core measures is required.
