PHYSICAL THERAPIST (PT) – LYMPHEDEMA / PELVIC FLOOR (PERMANENT, SNF)
Location: CA 91316
Start Date: ASAP
Schedule: Permanent (1 Position – Lymphedema or Pelvic Floor)
Compensation:
$50 – $58 per hour
Evaluate and treat SNF residents in Lymphedema or Pelvic Floor specialty
Provide skilled therapy services and documentation
Ensure Medicare/PDPM compliance
Collaborate with interdisciplinary care team
Maintain productivity and quality standards
Active Physical Therapist (PT) license
SNF or post-acute rehab experience required
Lymphedema or Pelvic Floor experience preferred
Knowledge of Medicare/PDPM and documentation standards
Strong communication and teamwork skills
