Prairie Wind Hospice
Prairie Wind Hospice seeks a skilled Nurse Case Manager to coordinate and deliver the highest level of hospice care to patients and families. The role collaborates with the Director of Nursing and the Interdisciplinary Team to assess needs, develop and update the Plan of Care, and coordinate services across the care continuum.
