Hybrid RN Case Manager - Hospitalist Discharge & Care

Stormont Vail Health in Topeka, KS is seeking a Hospitalist Case Manager to liaison with hospitalist teams for adult patients 18+. You will coordinate care, discharge planning, and ensure smooth transitions with community partners. Requirements include RN licensure in Kansas, BLS within 90 days, BSN preferred, and at least 5 years in acute hospital nursing.
