Hospice Case Manager

CHRISTUS VNA Hospice is seeking a Registered Nurse Case Manager to take full nursing responsibility for the Plan of Care for hospice patients and collaborate with the care team to promote physical, psychosocial, emotional, and spiritual well‑being.
