IRIS Program Consultant

Lutheran Social Services seeks an IRIS Program Consultant to support adults with long-term care needs in self-directing services that promote independence and community living. In this role, you’ll conduct assessments, develop person-centered service plans, educate participants on IRIS options and budgets, and coordinate care with healthcare, housing, and community providers.
