Work Environment\n

This position is fully remote, allowing for flexibility in work hours and locations while maintaining regular communication with clients and interdisciplinary teams. The role includes a structured schedule from Monday to Friday, 8:00 AM to 4:30 PM, with occasional evening or weekend hours required to accommodate client needs.

Key Responsibilities\n

The Licensed Clinical Social Worker will:

• Conduct thorough assessments of seniors' psychosocial needs, taking into account emotional, mental, and social variables.

• Develop and implement personalized care plans in collaboration with clients and their families, responding to their unique circumstances and aspirations.

• Provide counseling and emotional support to help seniors and their families navigate life challenges, including health-related transitions.

• Advocate for clients’ rights and access to resources, ensuring they are connected with appropriate community services and support groups.

• Collaborate effectively with multidisciplinary teams to enhance care coordination and client outcomes.

• Regularly monitor clients’ social and psychological conditions through home visits or virtual check-ins, addressing any emerging concerns.

• Facilitate communication among family members regarding care considerations, ensuring all voices are heard.

• Educate clients about available community resources, enhancing their engagement and social interaction.

• Maintain accurate client records, including assessments and progress notes, to ensure continuity and quality of care.

• Stay updated on current practices in social work, complying with ethical standards and regulations.

Qualifications\n

• Master's degree in Social Work from an accredited institution.

• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).

• A minimum of two years of direct experience in a clinical or social work setting, preferably with a geriatric population.

Compensation and Benefits\n

The salary range for this position is between $60,000 and $100,000 annually, commensurate with experience.
