HSS Coordinator - Northern, VA Market

The Care Coordinator will be the primary care manager for a panel of serious mental illness (SMI) members and may be assigned other health plan populations as needed. This position is responsible for completing Health Risk Assessments, Individualized Service Plans, and linking members to needed services to ensure that the members are receiving the healthcare they need. Responsibilities Complete Health Risk Assessments and Individualized Service Plans for assigned members. Link members to required services and coordinate care with physicians, behavioral health providers, and community stakeholders. Conduct face‑to‑face visits for 25–50% of assigned members. Support members’ medical, behavioral, and socioeconomic needs to promote appropriate utilization of services and improve quality of care. Collaborate and coordinate care with community partners such as the CSB. Work Location This is a field‑based position with a home‑based office. Candidates must reside within a commutable distance of Northern, VA. Remote work is possible as needed. Required Qualifications Bachelor’s degree in health or human services, LMHP, RN/LPN, QMHP, LMSW, LBSW, MSW, or BSW 1+ years of care coordination or behavioral health experience and/or work in a healthcare environment 1+ years of experience with MS Office, including Word, Excel, and Outlook Driver’s license, reliable transportation, and ability to travel within the assigned territory Preferred Qualifications CCM certification QMHP certification Experience working with Medicaid/Medicare populations Long term care or geriatric experience Experience working in team‑based care Background in Managed Care Benefits Hourly pay ranges from $23.89 to $42.69 per hour based on full‑time employment.
