The Licensed Practical Nurse (LPN) Ambulatory Care Advanced provides direct and indirect patient care under the direction of the Registered Professional Nurse, Nurse Practitioner, Physician's Assistant, or Physician. The LPN functions as part of the interdisciplinary health care team. The LPN, Advanced, serves as a resource for other team members and is active in the orientation of new staff. May be required to make home visits.

Responsibilities:

• Directs and supervises patient care provided by other LPNs and support personnel according to department guidelines.
• Identifies practice problems and consistently seeks solutions through appropriate communication channels.
• Demonstrates operational problem-solving ability through written exemplar, patient/family feedback, co-worker report or direct managerial observation
• Provides direct and indirect nursing care individualized to the patient's needs.
• Collects and records relevant patient health data accurately
• Evaluates health data to identify patient problems
• Implements and documents specified interventions
• Identifies deviations from expected patient outcomes
• Adheres to approved, research-based practice standards
• Performs and documents Point of Care (POC) testing and quality control in accordance with established laboratory policies and procedures
• Maintains and documents testing reagents/supplies and equipment according to established laboratory procedures
• Advocates for patients to ensure that care preserves and protects the patient's autonomy, dignity and rights.
• Delivers care in a non-judgmental and non-discriminatory manner that is sensitive to patient diversity
• Assures that patients have informed consent signed for indicated procedures
• Maintains current licensure, certification, and competencies.
• Provides proof of current BLS validation to supervisor.
• Completes required unit credentialing according to departmental policy
• Provides to supervisor evidence of completion all required annual training and competency validation
• Practices within the New York State-defined LPN scope of practice 100% of the time
• Acquires and maintains current knowledge and skills in his/her area of practice.
• Performs other duties as requested and observed by supervisor or manager.

Education:

• Graduation from a Practical Nursing program, required

Experience:

• Minimum 2 years' experience in relevant area of ambulatory practice, required

License/Certifications:

• Current NYS LPN Licensure as a Practical Nurse or a valid interim permit, required
• BLS certification with 6 months of hire, required
• Additional credentials as determined by specific work unit required

Physical Requirements:

• Heavy Work - Exerting up to 100 lbs. of force occasionally, and/or up to 50 lbs of force frequently, and/or up to 20 lbs, of force constantly to move objects
• Occasional climbing, crouching, grasping, kneeling, lifting, pulling, pushing, reaching, stooping, visual acuity (color, depth, perception)
• Frequent balancing, typing/ clerical/ dexterity, hearing, repetitive motion, seeing/ monitor/ computer use, standing, talking, walking
• Constant fit testing

Hazards:

• Occasional burns, chemical, electrical, mechanical, radiation/radiant energy
• Frequent bodily fluids/ bloodborne exposure

Working Conditions:

• Occasional noise, dust, fumes, gases, mists, odors

Travel:

• Occasional Travel - The job may require travel from time- to-time, but not on a regular basis

We are also offering a sign-on bonus for this position! The bonus amounts are based on the position type and are listed below. Additional details and eligibility requirements will be discussed during the interview process. • RN (excluding management) sign-on bonuses are awarded in the amount of $20,000 for full time
• LPN sign on bonuses are awarded in the amount of $10,000 for full time

Salary is based on a variety of factors, including, but not limited to, qualifications, experience, education, licenses, specialty, training, and fair market evaluation based on industry standards. The above compensation range represents a good faith belief of the compensation range by Bassett Healthcare Network, and its entities and affiliates, at the time of this posting or advertisement.

USD $25.74/Hr. USD $38.69/Hr.
