Responsibilities Assess and manage medical conditions for SNF patients under Physiatrist supervision Build individualized rehabilitation-focused care plans targeting function and quality of life Partner daily with physical therapy teams to coordinate patient recovery goals Prescribe and manage medications in support of pain control and rehabilitation outcomes Track patient progress and refine care plans based on functional milestones Provide clear, compassionate education to patients and families throughout the care journey Complete clinical documentation accurately and on time per regulatory standards Qualifications Active, unrestricted Pennsylvania NP license ( AGPCNP or FNP preferred) MSN required 1–3 years NP experience; skilled nursing facility background strongly preferred Natural collaborator comfortable in a multidisciplinary rehabilitation environment Independent caseload management skills with strong clinical judgment Low tolerance for unnecessary administrative burden — this practice shares that value Schedule & Compensation $120,000–$135,000/year + RVU productivity bonus Full-time | Monday–Friday only No weekends | No on-call | No after-hours charting — guaranteed Flexible schedule design — be home for dinner every night 401(k)/403(b) | PTO/sick days | Malpractice covered Full in-person onsite training provided Immediate hire Hiring now!
