Description

• Assess patients in either inpatient or outpatient settings and create customized treatment plans
• Deliver physical therapy interventions focusing on improving mobility, strength, balance, and overall functionality
• Work in partnership with nurses, physicians, case managers, and other rehabilitation professionals to enhance patient outcomes
• Guide patients and their families through therapeutic objectives, discharge planning, and home exercises
• Accurately document evaluations, interventions, and patient progress in accordance with facility and regulatory protocols
• Engage in care team rounds and interdisciplinary meetings to ensure seamless care transitions

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification is preferred
• Specialized certifications (e.g., neurological, geriatric) are advantageous but not necessary

Skills

• Prior experience preferred
• Adeptness in treating patients with intricate medical conditions and mobility challenges
