Outpatient Rehab Case Manager - Neuro & Medically Complex

SSM Health Day Institute in Olivette, MO is seeking a Case Manager to work in our outpatient rehabilitation program with the ability to float to other Day Institute facilities across the St. Louis region. Under general direction, you will monitor and assess patient plans from pre-admission to post-discharge, assist families in goal setting to maximize independence, and liaise with payers to obtain necessary authorizations for initiating and continuing care throughout admission to the program. #J-18808-Ljbffr Select Medical
