Social Worker (Mental Health Consultant)

Duties

Help

Provide targeted assessment and evaluation of presenting problem.

Formulate evidence-based interventions for a wide range of mental health conditions.

Collect bio-psycho-social information about patients.

Document care and recommendations concisely in the medical record chart.

Refer patients to Specialty Mental Health Care as needed.

Participate in program activities such as huddles, provider meetings and trainings.

Function as an integrated full-time team member in the primary care.
