Job Summary This position offers an opportunity to be part of a dynamic PHP/IOP program located at the Institute of Living. The case manager will be directly involved with assisting patients locate resources in the community and on campus, apply for additional benefits, access local transportation services to facilitate program adherence and complete paperwork for leave status.
