POSITION SUMMARY - LCSW Social Worker


POSITION DUTIES - Provide clinical assessment, crisis intervention, care coordination, and discharge planning to help patients and families navigate medical, emotional, and social needs during hospitalization.


MINIMUM REQUIRED QUALIFICATIONS -

• CO LCSW
• BLS
• 2 years social work experience in an acute hospital setting
• Acute hospital experience in specialty during the past year with no more than a 90-day break

PREFERRED QUALIFICATIONS -

• ED experience
• Palliative Care Knowledge and GOC Conversation
• Behavioral Health experience with suicide risk and substance use assessments
• Discharge planning experience

LENGTH OF ASSIGNMENT - 13 weeks


SHIFT / HOURS PER WEEK - 8A - 4:30P with some weekends


SYSTEMS - Meditech/Midas/Navihealth


START DATE - 8/3/2026
