Discharge planning experience Job Description & Requirements LCSW Social Worker StartDate: 8/17/2026 Pay Rate: $1600.00 - $2500.00 POSITION SUMMARY – LCSW Social Worker Provide clinical assessment, crisis intervention, care coordination, and discharge planning to help patients and families navigate medical, emotional, and social needs during hospitalization. Minimum Required Qualifications – CO LCSW BLS 2 years social work experience in an acute hospital setting Acute hospital experience in specialty during the past year with no more than a 90-day break Preferred Qualifications – ED experience Palliative Care Knowledge and GOC Conversation Behavioral Health experience with suicide risk and substance use assessments Discharge planning experience LENGTH OF ASSIGNMENT 13 weeks SHIFT / HOURS PER WEEK 8A - 4:30P with some weekends SYSTEMS Meditech/Midas/Navihealth
