Job TitleProvides leadership and supervision of the day-to-day clinical and administrative operations of the care management team.Job DescriptionSupervises Care Management (CM) staff to include nursing, social work, patient care coordinators and other non-clinical staff to achieve the goals for the organization; collaborates directly with inpatient and ambulatory clinicians and care teams, as well SMG and PCN leaders to support cost-effective high-quality medical services.Providence caregivers are not simply valued – they're invaluable.
