Job Summary

Cuready Healthcare Staffing is seeking a qualified RN Case Manager to support patient care and clinical operations within healthcare facilities. The ideal candidate will deliver safe, high-quality care while collaborating with interdisciplinary care teams and maintaining compliance with facility policies and professional standards.

Responsibilities

• Coordinate and manage comprehensive patient care plans.
• Conduct needs assessments and determine medical necessity using evidence-based guidelines.
• Collaborate with healthcare teams to ensure quality care and resource utilization.
• Utilize Epic for charting and documentation while adhering to facility protocols.
• Facilitate discharge planning activities and ensure continuity of care.
• Participate in case reviews to optimize patient outcomes and care efficiency.

Qualifications

• Active AK RN License required upon submission.
• BLS certification preferred.
• Minimum 1 year of acute care hospital experience required.
• Familiarity with CMS, ICD-10 coding, and care coordination.
• Experience with Epic charting system required.
• ACM certification and Microsoft Office/Teams experience preferred.
