QUALIFICATIONS:

Graduate of a vocational/technical nursing program Current LVN/LPN license in the state of practice Knowledge of lab procedures, diagnostic testing, and EMR systems Strong documentation, communication, and organizational skills

KEY RESPONSIBILITIES:

Review medical records, identify gaps in care, and schedule preventive services per physician-approved protocols. Collaborate with providers, Nurse Navigators, and families to ensure continuity of care and patient support. Educate patients using evidence-based guidelines to promote engagement and health compliance.
