Pediatric Case Manager - MSW/LCSW

Pediatric Case Manager - MSW/LCSW HealthCare Support is actively seeking a Pediatric Case Manager - MSW/LCSW to fill an opening with a Fortune top 25 Managed Care Company in Sunrise, FL. This is a hybrid role working remote and in office on Thursdays in Sunrise, FL 33323. Additionally, this position may require some field work, such as home or skilled nursing facility visits. Daily Responsibilities Mon‑Fri: 8:30 a.m.–5 p.m. Identify special‑needs members through the completion of health screens and other resources. Work with community outreach/member advocates to coordinate member care. Educate providers and community resources on program components and available support services. Educate members with special needs to foster compliance with program and positively impact outcomes, helping complete the monthly and quarterly reports that come with that transition. Home visits may be required for discharge assessments depending on the situation. Required Qualifications Masters of Social Work (MSW) required. 2+ years of experience as a social worker managing and integrating the social/community needs of members. Experience with Medicaid Program and CMS Guidelines. Knowledge of Microsoft Office and internet software. Ability to work telephonically. Strong documenting experience in multiple systems. Preferred Qualifications LCSW highly preferred. Bilingual in English/Spanish. Behavioral health experience preferred. Managed Care/Health Plan experience highly preferred.
