Participates in the coordination of care. Job Classification Clinical Lines of Authority Reports to Director of Nursing Job Qualifications Education Masters or doctoral degree from an accredited program in social work. Licensure Certification. Current driver’s license. Experience Minimum of 1 year of hospice experience required as a Medical Social Worker (MSW), with strong skills in patient support and end-of-life care coordination. Skills Working knowledge of community resources. Good interpersonal skills. Proof of current CPR certification. Fluent in both English and Spanish (spoken). Transportation Reliable transportation and valid auto liability insurance. Environmental and Working Conditions Works in patients’ homes in various conditions; possible exposure to infectious diseases; ability to travel locally; some exposure to unpleasant weather. Physical and Mental Effort Prolonged standing, walking, and sitting required.
