Job Description

The Outpatient Physical Therapist will evaluate all assigned patients and develop, implement, and modify treatment plans in accordance with the patient’s needs, goals, and physician direction. This role is responsible for the delegation and direction of all discipline-specific therapy services for those assigned patients, ensuring the design of functional treatment programs that facilitate effective return to community activities. The therapist also serves as an educator to patients, families, students, coworkers, and themselves while adhering to the organizational mission and core values.

Responsibilities

• Complete a comprehensive evaluation of assigned patients.
• Utilize clinical reasoning to develop appropriate treatment plans and goals.
• Document medical records clearly, accurately, and in a timely manner.
• Provide care under signed, valid physician orders within insurance approval.
• Participate in student programs, creating a positive learning environment.
• Actively pursue professional development and maintain licensure requirements.
• Engage in performance improvement initiatives to enhance service quality.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 1+ year of outpatient clinical experience.
• CPR Certification.
• Active and applicable state licensure.
• Previous assignments with the agency are highly desired.
