As a vital member of our healthcare team, the Admissions Concierge plays a crucial role in ensuring a seamless transition for new patients at Pearl Nursing Center of Rochester LLC. Our mission is to provide exceptional care and exceptional service, and we're seeking a compassionate and organized individual to join our team.
Responsibilities:
Assess and coordinate patient admissions, discharges, and transfers in a timely and efficient manner
Communicate effectively with patients, families, and healthcare providers to gather necessary information and address concerns
Collaborate with interdisciplinary teams to develop and implement individualized care plans
Maintain accurate records and documentation to ensure compliance with regulatory requirements
Provide exceptional customer service to patients, families, and staff, demonstrating a commitment to our organization's values
About the Role:
As an Admissions Concierge, you will have the opportunity to make a lasting impact on the lives of our patients and their families. You will work closely with our healthcare team to ensure a smooth transition from hospital to home and provide ongoing support to patients and families throughout their care journey.
What We're Looking For:
1 years of experience in skilled nursing administration or a related field
High school diploma or equivalent required; degree in healthcare administration, business, or a related field preferred
Excellent communication and organizational skills with the ability to work effectively in a fast-paced environment
Empathy and compassion for patients and families, with a commitment to providing exceptional customer service
Ability to maintain confidentiality and handle sensitive information with discretion
