LPN Home Health

Job Summary:

The Licensed Vocational Nurse (LPN) in home health care is responsible for providing skilled nursing care during home visits to patients in the Vermont territory. This role involves performing patient assessments, medication administration, and collaborating with the interdisciplinary team to deliver outpatient care. The LPN must manage schedule changes, ensure compliance with clinical protocols, and maintain accurate documentation using EMR systems. Strong communication skills and a valid driver's license are essential for effectively traveling between patient homes and delivering quality care. Responsibilities:

• Conduct patient assessments and provide skilled nursing care during home visits.
• Administer medications and treatments as prescribed and documented.
• Collaborate with interdisciplinary team members to develop and update patient care plans.
• Document patient care and visit details accurately in the EMR system (HealthWyse/Careficient).
• Manage and adapt to schedule changes and patient needs in a dynamic outpatient environment.
• Ensure compliance with home health and hospice care standards and protocols.
• Maintain communication with patients, caregivers, and healthcare providers.
• Travel to patient homes safely and efficiently within the assigned territory.

Required Skills & Certifications:

• Current Licensed Vocational Nurse (LPN) license in good standing.
• Minimum of 2 years experience in home health and hospice care.
• Valid driver's license with a clean driving record.
• BLS (Basic Life Support) certification.
• Experience with electronic medical records systems, preferably HealthWyse/Careficient.
• Ability to manage and adapt to schedule changes and patient care priorities.

Preferred Skills & Certifications:

• Experience in outpatient care settings.
• Familiarity with hospice care protocols and interdisciplinary team collaboration.

Special Considerations:

• Some weekend coverage is required.
• On-call duties may be scheduled but only on an emergency basis.
• Work involves travel to multiple patient homes within the Vermont territory.

Scheduling:

• Monday through Friday, 8:00 AM to 5:00 PM day shifts.
• Occasional weekend shifts and emergency on-call coverage.
