Care is typically delivered within 2 hours of receiving a request, emphasizing fast access, accurate results, and a seamless patient experience. Role Description This is a part-time, hybrid Family Nurse Practitioner role based in Queens, NY, with a combination of in-person visits and work-from-home telehealth hours. The Family Nurse Practitioner will provide comprehensive primary and urgent care through virtual visits and in-home encounters, including assessment, diagnosis, treatment, and follow-up for patients across the lifespan. Daily responsibilities include conducting evidence-based evaluations, ordering and interpreting labs and rapid tests, prescribing medications, and coordinating care with on-site medical assistants. The role also involves documenting encounters in the electronic medical record, supporting clinical protocols for telehealth and home visits, and contributing to continuous improvement of Zoomdoc’s care pathways. The clinician will collaborate with a multidisciplinary team to deliver safe, high-quality, and timely care that aligns with Zoomdoc’s commitment to accessible, patient-focused service. Qualifications Current, unrestricted Family Nurse Practitioner (FNP) or similar Nurse Practitioner licensure. Clinical background in Medicine and Urgent Care, with the ability to manage common acute conditions and determine appropriate escalation of care. Experience or strong interest in telehealth, home-based care, and use of digital health tools and remote diagnostics. Active NP license in New York state. Current DEA registration and compliance with all applicable state and federal prescribing regulations. Exemplary clinical judgment, clear written and verbal communication, and comfort working independently in a hybrid model. Basic proficiency with electronic medical records and telehealth platforms; willingness to learn new clinical technologies.
