About the Role

The Hospice and Home Health Community Liaison is responsible for developing and maintaining referral relationships, increasing awareness of hospice and home health services, and supporting sustainable census growth. This position connects the agency with hospital departments, physicians, skilled nursing facilities, assisted living communities, and other healthcare and community partners. The role involves regular travel throughout the service area, community outreach, education, referral follow-up, and collaboration with clinical and intake teams to support appropriate and timely referrals. Frequent interaction with healthcare professionals and community partners is an important part of building strong relationships and increasing awareness of hospice and home health services.

Minimum Qualifications

· Bachelor's degree in marketing, business, healthcare, nursing, social work, or a related field preferred; relevant healthcare marketing or business development experience may be considered in place of a degree.

· Healthcare marketing, business development, hospice, home health, hospital, physician relations, or post-acute healthcare experience preferred.

· Excellent communication, presentation, relationship-building, and organizational skills.

· Ability to work independently, manage competing priorities, and represent the organization professionally in community settings.

· Valid driver's license, reliable transportation, and appropriate automobile insurance.

Preferred Qualifications

· Experience developing relationships with healthcare referral sources.

· Working knowledge of hospice and home health services, eligibility, and referral processes.

· Experience with referral tracking, reporting, presentations, community education, or healthcare outreach.

· Knowledge of Medicare hospice and home health eligibility requirements.

Responsibilities

· Develop and maintain relationships with physicians, hospitals, clinics, nursing facilities, assisted living communities, rehabilitation facilities, and other potential referral sources.

· Conduct regular face-to-face visits with current and prospective referral partners.

· Educate healthcare professionals and community organizations about hospice and home health services, eligibility, benefits, and referral processes.

· Promote appropriate and timely referrals to hospice and home health.

· Work with hospital case management, discharge planning, the emergency department, physician practices, and other departments to identify patients who may benefit from services.

· Follow up on referrals and maintain communication with referral sources while protecting patient confidentiality.

· Identify new referral opportunities and develop strategies to support hospice and home health admissions and census growth.

· Track referral activity, referral sources, admissions, conversion rates, and outreach efforts.

· Participate in community events, health fairs, educational programs, networking events, and professional organizations.

· Assist with marketing materials, presentations, outreach campaigns, and community education.

· Coordinate with clinical and intake teams to ensure outreach accurately reflects available services.

· Maintain professional relationships with referral partners and represent MRHC positively in the community.

· Follow HIPAA, organizational policies, Medicare requirements, and applicable federal and state regulations.

The Hospice and Home Health Community Liaison uses strong communication, presentation, relationship-building, and organizational skills to develop referral partnerships and represent MRHC in the community. The position requires the ability to communicate hospice and home health services accurately to healthcare professionals and community organizations. Strong follow-through, attention to detail, and independent judgment are important when managing outreach activities, referrals, and competing priorities. The liaison must also be comfortable tracking referral activity, preparing reports, participating in community events, and working closely with clinical and intake teams while maintaining patient privacy and regulatory compliance
