Case Manager-Adult Medical Day Program

Total Care Services, Inc. (TCS) is seeking a Case Manager for Adult Medical Day Care Services (MDCSW) to support individuals with complex medical and functional needs. In this role, you will help clients receive quality care in community-based settings rather than institutional environments. With over 21 years of service, TCS is a nonprofit organization committed to person-centered care, regulatory excellence, and community impact. Key Responsibilities Conduct assessments using Person-Centered Planning (PCP) Develop and manage individualized service plans Coordinate Adult Medical Day Care and community-based services Document all service coordination activities in LTSS Maryland daily Conduct in-person client visits Monitor service effectiveness and client outcomes Ensure compliance with program and regulatory requirements Maintain accurate and timely documentation Qualifications Required: Bachelor’s degree in Human Services or related field (or approved equivalent) Some experience in case management or service coordination Strong communication, documentation, and organizational skills Valid driver’s license, reliable transportation, and safe driving record Ability to travel locally throughout Maryland Must be at least 21 years old Ability to pass background checks Authorized to work in the United States Preferred: Experience with Adult Medical Day Care or Medicaid Waiver programs Familiarity with LTSS Maryland system Experience working with individuals with complex medical or behavioral needs Schedule Full-time, Monday through Friday Flexible based on client needs About Total Care Services, Inc.
