Position Licensed Practical Nurse (LPN) - Home Health Location: Orrville, OH Job Id: 12863 # of Openings: 1 Role Overview Absolute Home Health is currently seeking a compassionate Skilled Licensed Practical Nurse (LPN) to assist clients one-on-one in their homes. Our goal is to assist our patients with transitioning home from the hospital and provide continuous care, needed to remain at home with their loved ones. Our LPNs are an essential part of the team. The Licensed Practical Nurse is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes. Absolute Home Health provides skilled nursing, therapy services, social services, and home health aides in the home under the direction of the patient's primary care physician. Responsibilities Provide direct patient care under RN supervision. Follow patient care plans and provide treatments. Collaborate with interdisciplinary team members. Document clinical progress notes and maintain care records. Adhere to policies and procedures. Job Requirements Work well in a team environment and independently Demonstrates flexible and efficient time management and ability to prioritize workload Computerized medical records Provides skilled nursing care to clients as directed by the registered nurse and physician’s plan of care. Adherence to policies and procedures. Helps to achieve and maintain continuity of client care by assisting in planning and exchanging information with other health professionals. Prepares clinical progress notes and follows client care plans as developed by the registered nurse and according to agency policy.
