Community-Based Children's Case Manager (Sign-On Bonus)

The Guidance Center in Michigan is seeking a Case Manager to provide assessment, linking, coordinating and monitoring services for children and families. Responsibilities include treatment planning, family education, transportation coordination, and community-based supports. Work hours are Monday through Friday 8:30–5:00 with 4 hours after 5pm weekly; weekends may be required.
