The successful candidate will provide in-home therapy services primarily to adult patients, focusing on orthopedic rehabilitation, surgical recovery, wound care, and other multidisciplinary cases.Shift Details\n \n Shift: Monday to Friday\n Hours: 8:30 AM to 5:00 PM\n Guaranteed Hours: 40 hours per week\n Responsibilities\n \n Deliver in-home therapy to patients, including admissions, existing patients, and recertifications.\n Collaborate in multidisciplinary meetings held on Mondays and Wednesdays to discuss current admissions.\n Manage patient caseload of 4-6 patients per day.\n Provide therapy services within a designated geographic area, with limited oxygen requirements.\n Utilize Epic EMR for documentation and patient record keeping.\n Qualifications\n \n Minimum of 1 year of experience as a licensed Physical Therapist, with a preference for home health experience.\n Experience in orthopedic therapy is required.\n BLS Certification from AHA is mandatory.\n First-time travelers are welcome to apply.\n Other Information\n \n No mileage reimbursement is offered; the position involves travel within a 50-mile radius, both east and west, and up to 60 miles north and 40 miles south.\n Business casual attire is expected, with any color scrubs permitted.\n Orientation includes Epic classes and shadowing up to 1 week with another therapist.\n \n \n Wellspring Nurse Source Job ID #37416560.
